1. Thrive Rewards is a discount program. Participating in Thrive Rewards is only permissible in accordance with the Terms. Member must fully and accurately report the stated discount price, or if applicable, any net pricing, after giving effect to any rebates, to Medicare, Medicaid, Tricare and any other federal or state program upon request by any such program. Accordingly, Member should retain related purchase documents and these Terms for Member's records. It is the Member's responsibility to review any agreements, invoices, or other documents applicable to these prices to determine if Member's purchases are subject to a discount or rebate. Some exclusions may apply and are subject to change. Arestin, Zyris Inc. products, Discontinued/Undefined Products, Pharmaceutical—RX, Vaccines, Medical Products (Equipment, Equipment Rental, Extended Service Warranty), Veterinary Products (Equipment, Equipment Rental, Extended Service Warranty), Podiatry Products (Equipment, Equipment Rental, Extended Service Warranty), Refurbished Equipment, Dentrix (Hardware, Software, Services, Warranty, Parts & Labor), Deferred Service Contract Fees and Storage Fees are not eligible.
